Georgia Families 360
Georgia Families 360°SM is a program that delivers care to children, youth and young adults in foster care; children and youth receiving adoption assistance; and certain youth in the juvenile justice system. Amerigroup Community Care is one of the health plans that delivers care management services for the Georgia Families 360°SM program.
Georgia Pathways
Pathways is a new program that gives low-income Georgians 19-64 who are not eligible for Medicaid today a new opportunity to gain access to healthcare coverage through Medicaid. Georgia residents can apply for the program starting on July 1, 2023.
Pathways members are eligible to receive the same State Plan benefits as other Medicaid groups, with the exception of nonemergency medical transportation (NEMT). Only members ages 19 and 20 may access NEMT, dental and vision services as part of their EPSDT.
Behavioral health
At Amerigroup, our behavioral health care benefit is fully integrated with the rest of our health care programs. Amerigroup works collaboratively with hospitals, group practices and independent behavioral health care providers, as well as community agencies, Georgia’s community service boards and other resources to successfully meet the needs of members with mental health and substance use conditions, and/or intellectual and developmental disabilities.
Maternal child services
When it comes to our pregnant members, we are committed to keeping both mom and baby healthy. That’s why we encourage all our moms-to-be to take part in our Taking Care of Baby and Me® program, a comprehensive, proactive case management and care coordination program for all expectant mothers and their newborns. As part of the Taking Care of Baby and Me program, members are also offered the My Advocate® program. This program provides pregnant and postpartum women proactive, culturally appropriate outreach, and education through Interactive Voice Response. Eligible members receive regular phone calls with tailored content from a voice personality (Mary Beth) or they may choose to access the program via a smartphone application or website.
Disease Management
Our Disease Management (DM) team is based on a system of coordinated care management interventions and communications designed to assist physicians and others in managing members with chronic conditions.
